Insurance we accept: We participate with Medicare, Medicaid, Aetna, Americhoice, Amerigroup, Amerihealth, Avesis, Blue Cross/Blue Shield, Cigna, Clover, Davis Vision, Eye-Med, HealthNet, NVA, Oxford, Qualcare, Spectera, Super Vision Plan, UnitedHealthcare, Vision Benefits of America and VSP.

Policies

Dr. Fruchtman’s will charge a $50 cancellation fee when a patient fails to provide us with at least 48 hours’ notice. By not giving us ample notice, we’re not able to help other patients. Thank you for understanding.

We understand life happens. If you’re going to be late, please call us. If you don’t call us, and are 15 minutes late or more for your appointment, the appointment will need to be rescheduled, and a $50 reschedule fee will be charged.
